Care4Diabetes App is developed for users with Type 2 diabetes in order for them to record, monitor and manage their diet, blood glucose level and intake of medication.
The content of the App is divided into sessions of education, dietary and medication logs, blood glucose logs, individualised reminder and messaging.
Education session includes reading topics and videos from the basic knowledge and self-management of diabetes to relaxation exercises for the users.
The App users are able to update their blood sugar levels before or/and after their meals follow by diet and medication inputs in which provides a record for them to keep track of their progression in diabetes self-care.
Reminder and messaging function allows the users be reminded to attend to certain events and to communicate with their healthcare professionals instantly.
