This mobile application allows users to fill out Environmental and HSE Observation Reports. Users can fill out incident information (e.g. involved persons, witnesses, exact location, etc.), description of unsafe acts, along with the photo of the unsafe act, and the cause of the unsafe act. Additionally, users can record the immediate action that was taken to correct the unsafe act. Users can also record the employee who was responsible and who reported the unsafe act. Users can customize this application to fit their own HSE and environmental Observation Reports.
