DEARhealth is a new solution enabling you securely manage your health. Use your DEARhealth account to manage your health and disease activity under the principles of Value-Based healthcare.
We measure the value of your care with a Value Quotient® or VQ® which captures the value of healthcare services to individual patients, including disease control, quality of life and daily productivity.
DEARhealth allows you to:
• Check your current and historical VQ®
• View your individualized Care Program
• Complete your home care
• Contact your healthcare team via secure messaging
• View upcoming labs, clinic visits, scheduled home care, and procedures
• Learn about your disease with MyAcademy
• Access your Wellness programs - MyCoach, MyMenu, MyAccupressure, My Fitness, MyYoga and MyMeditation.
• Review and confirm your medication
To access the DEARhealth services delivered through the app, you must create an account with your healthcare provider. Visit DEARhealth website for a listing of supported providers. If your provider is not listed or you need help setting up the account, please contact them directly.
