Let's say you're driving a car and you switch on the driving session by setting the end time.
On the first call from a number x, your phone will be silent and an SMS will be sent to x that you're driving and will be available after a "set time".
If x calls again, your phone will vibrate and another SMS will be sent. BTW you can customize the SMS.
The third time, the call from x will ring, which is an indicator of emergency and you can act accordingly.
If y or any new number calls, the same procedure will take place.

The SMS permission is asked for sending SMS to the callers when you can't pick calls. The cases where a user's physical interaction with the device is limited.
The DND permission is asked for changing the DND state from silent to vibrate or to ring.
